Here's the one for my show (initially mentioned over in Pub section). It was designed by the actor who's playing Jack--he's a graphic designer by profession.

CYAC stands for Contemporary Youth Arts Company. The "Scarpelli" is composer Mark Scarpelli and "Kehde" is lyricist/director Dan Kehde.
